A simple, six-step pathway designed to minimise your administrative burden while keeping you informed at every stage.

Send a Referral

Submit a referral via email (info@fortifymovement.com.au), fax, or phone. A standard GP/specialist referral letter, Medicare referral form, or brief clinical summary is all we need. DVA referrals welcome; we also welcome self-managed and plan-managed NDIS participants.

We Contact Your Patient

Our team will contact your patient within 1–2 business days to schedule an initial consultation. We'll confirm receipt and let you know the appointment has been booked.

Initial Consultation & Assessment

Your patient attends a comprehensive initial assessment with one of our Accredited Exercise Physiologists. We review their clinical history, goals, capacity, and any relevant investigations.

Personalised Exercise Program

An individualised, evidence-based exercise program is designed and initiated, aligned with the referring clinician's goals and the patient's medical context.

Clinical Communication

A written clinical summary is sent back to you following the initial consultation. Ongoing progress reports are provided at key intervals, or on request.

Ongoing Review & Discharge

We monitor progress against measurable outcomes and communicate milestones or changes in clinical status. Discharge summaries are provided upon program completion.

Referral Criteria & Funding Pathways

We see a broad range of presentations. Below is a guide to appropriate referrals and the funding options we accept.

Suitable for Referral

  • Chronic musculoskeletal pain (back, neck, joint pain)
  • Osteoarthritis, osteoporosis, inflammatory arthritis
  • Type 2 diabetes and metabolic syndrome
  • Cardiovascular disease, hypertension, heart failure
  • Chronic obstructive pulmonary disease (COPD) and respiratory conditions
  • Obesity and cardiometabolic risk
  • Cancer — prehabilitation and post-treatment recovery
  • Neurological conditions: stroke, MS, Parkinson's, acquired brain injury
  • Mental health conditions with exercise as an adjunct therapy
  • NDIS participants — physical, psychosocial, and neurological disabilities
  • DVA Gold & White Card holders
  • General deconditioning, falls risk, and healthy ageing
  • Post-surgical or post-hospital rehabilitation

Not Currently Suitable

  • Acute orthopaedic injuries requiring immediate surgical review
  • Unstable cardiac or respiratory conditions not yet medically managed
  • Patients who require physiotherapy-specific manual therapy interventions
  • Patients awaiting acute specialist workup where exercise is contraindicated

NDIS & DVA Funding Supported

Supporting self-managed and plan-managed NDIS participants, and supporting DVA clients. Your patients can access funded services without financial barriers.

Specialist Assessment Tools

Objective assessments using AxIT force-plate and dynamometry testing, Tanita body composition analysis, and validated clinical outcome measures.

Multidisciplinary Approach

We work collaboratively with GPs, specialists, psychologists, dietitians, and community supports to deliver coordinated, patient-centred care.

All Fortify Movement & Health clinicians are Accredited Exercise Physiologists (AEP) credentialled by Exercise & Sports Science Australia (ESSA) — recognised as allied health professionals under Medicare, NDIS, DVA, WorkCover, TAC, and most private health funds.
