We welcome self-managed and plan-managed NDIS participants, offering accredited exercise physiology and clinical rehabilitation services at our Altona North, Victoria clinic. We work with NDIS participants, their families, and support coordinators to ensure a clear, supported pathway from plan to program.

How to Access Services Through Your NDIS Plan

Check Your NDIS Plan

Review your NDIS plan to confirm you have funding under the "Improved Health & Wellbeing" or "Capacity Building" support categories. Our clinicians can help clarify which category applies to your goals.

Obtain a Referral or Self-Refer

Self-managed and plan-managed participants may self-refer directly or obtain a referral from your GP or support coordinator.

Initial Clinical Assessment

Your Accredited Exercise Physiologist will conduct a comprehensive clinical assessment to establish your baseline, identify your goals, and design an evidence-based exercise program tailored to your needs.

Begin Your Program

Start your individual or group exercise sessions at our Altona North clinic. Progress is monitored and documented, with regular reviews to ensure your program aligns with your NDIS goals.

How to Access DVA-Funded Exercise Physiology

Step 1

Obtain a DVA Referral

Ask your GP or treating medical officer for a referral to an Accredited Exercise Physiologist. DVA white card and gold card holders may be eligible for services under specific treatment pathways.

Step 2

Confirm Your Eligibility

DVA-funded services are available to eligible veterans, war widows, and dependants. Our team can help clarify which DVA treatment pathways apply to your health needs — no obligation.

Step 3

Begin Your Programme

Once your referral is in place, we design an evidence-based exercise physiology programme tailored to your specific condition, goals, and service history. No gap fees where DVA coverage applies.
