How Exercise Helps

The kidneys don't work in isolation. Exercise creates positive ripple effects across the whole body — including the systems that put the most pressure on renal health.

Blood Pressure Control

Structured aerobic exercise has been shown to reduce systolic blood pressure by 4–6 mmHg in people with CKD — a meaningful improvement that eases strain on the kidneys and the heart. Consistent movement helps the vascular system work more efficiently, reducing the haemodynamic load that accelerates renal decline.

Cardiovascular Risk Reduction

CKD significantly elevates cardiovascular mortality risk. Regular exercise improves lipid profiles, reduces inflammation, and enhances cardiac output — each of which chips away at the excess CV risk that comes with impaired kidney function. The evidence is clear: physically active CKD patients have better cardiovascular outcomes.

Phosphate Management

Exercise supports improved metabolic function, which can complement dietary phosphate management. While exercise alone doesn't replace dietary strategies, improvements in muscle metabolism and insulin sensitivity contribute to better overall mineral handling — a key consideration as CKD progresses through its stages.

What to Aim For

Exercise guidelines for CKD are adjusted by stage. The table below reflects current clinical recommendations — your accredited exercise physiologist will fine-tune these to your individual kidney function, comorbidities, and goals.

Staying Safe

Exercise and CKD can absolutely go hand in hand — with the right precautions. Here's what your clinical exercise program will keep a close eye on.

eGFR & Creatinine Monitoring

Regular blood work is essential. Your accredited exercise physiologist works alongside your nephrologist and GP to review eGFR and creatinine trends before adjusting exercise intensity or volume. A sudden drop in eGFR warrants a clinical review before continuing a progression.

Electrolyte Awareness

Impaired kidney function affects potassium, sodium, and phosphate regulation. Exercise causes electrolyte shifts, so it's important to be aware of symptoms such as muscle cramps, unusual fatigue, or heart palpitations. Pre-exercise bloods help guide safe exercise parameters, particularly in later stages.

Fluid & Protein Considerations

Hydration needs are more nuanced for people with CKD — especially if fluid restrictions are in place. High-protein exercise supplementation common in general fitness settings is typically unsuitable. Exercise programming accounts for your specific dietary and fluid restrictions to keep sessions safe and effective.

Dialysis Coordination

For those on haemodialysis or peritoneal dialysis, exercise timing is key. Intra-dialytic exercise (during HD sessions) is well-supported in the literature, and our team can liaise with your dialysis unit. Post-dialysis fatigue should be considered when scheduling sessions on non-dialysis days.
