While there is no cure, a growing body of evidence shows that carefully prescribed exercise is one of the most effective non-pharmacological treatments available — reducing pain, improving sleep, and meaningfully lifting quality of life. Exercise isn't just "good for you" with fibromyalgia — it's one of the most powerful tools available. Here are the five key mechanisms behind its benefits.

Pain Centralisation

Fibromyalgia involves central sensitisation — the nervous system becomes hyper-responsive to normal inputs, amplifying pain signals. Regular aerobic exercise has been shown to down-regulate central sensitisation pathways, reducing the nervous system's over-response to pain. Over time, consistent movement can meaningfully lower widespread pain scores and increase pain tolerance.

Why "just exercise more" isn't the answer

Unsupervised or poorly dosed exercise can worsen fibromyalgia symptoms — which is why so many people with the condition have tried exercise and stopped. Staying Safe

Exercise is safe and highly recommended for fibromyalgia — but how you approach it matters enormously. These four principles help keep you moving forward without unnecessary setbacks.

Incremental Loading

One of the most important principles when exercising with fibromyalgia is 'start low, go slow'. Beginning with very short, gentle sessions — even 5–10 minutes — and increasing duration and intensity by no more than 10% per week helps the nervous system adapt without triggering a flare. Your AEP at Fortify will build your program in stages, reassessing regularly to ensure progress is sustainable.

Symptom Tracking

Keeping a simple daily log of pain levels, fatigue, sleep quality, and mood gives your exercise physiologist invaluable data. Patterns in your symptom diary help identify which activities, times of day, or intensities suit you best — and which ones to modify. Fortify's AEPs use this information to fine-tune your program and spot early signs of overload before they become a setback.

Rest–Activity Balance

The 'boom-bust' cycle — overdoing activity on a good day and crashing afterward — is a common and counterproductive pattern in fibromyalgia. Pacing strategies help you spread activity evenly across the week, take planned rest breaks, and set an 'activity ceiling' to protect your recovery. Building rest into your program is as clinically important as the exercise itself.
